This form is a mandatory form which complies with the requirements for posting on the Family Violence-Stalking Registry, GCIC and the NCIC computer networks.

Therefore, the form should not be edited without the strictest attention to detail. It is far better to add paragraphs if modifications are required.

Certain provisions are mandatory. The specific paragraph numbers assigned to mandatory provisions are those numbered paragraphs which have a "subscript" number adjacent to it. Neither these paragraph numbers, nor their accompanying mandatory provisions can be modified as these numbered paragraphs and their provisions are entered "verbatim" upon the TPO registry. Furthermore, any reformatting of this document must leave intact the "subscript" numbers as these relate to entry upon the registry.

Non-mandatory provisions do not have an adjacent "subscript" number. They may be "reasonably" edited with an eye towards caution.
However, again, please do not change the numbering of paragraphs as the numbering directly relates to specific provisions on the TPO-Stalking Protection registry.

Also, the form requires that certain provisions are ONLY effective if "initialed" by the judge. Therefore, consent orders are not valid when these particular provisions are simply "initialed" by counsel or the parties. Please make certain that consent orders provide for "initialing" by the judge and please make certain the judge initials those provisions. That way the order can be properly docketed with the court and properly placed upon the registry for protection of party(s).
